Our GIFTS Program (Giving Incredible Families Tools for Success) provides free, voluntary short-term case management services specifically designed for Wisconsin families like yours.
What Is Short-Term Case Management?
Short-term (up to 6 months) case management is professional, individualized support that helps connect your family to the right resources and services. Unlike ongoing therapy or counseling, our case management focuses on practical solutions and parent coaching to address your family’s immediate needs.
Our Case Management Services Include:
- Providing ongoing support via phone, online, or in-person
- Administering family needs assessments and parent/caregiver stress assessments
- Co-creating plans to work on targeted concerns
- Providing individualized parent/caregiver skill development & coaching to increase caregivers’ skill levels
- Making service referrals based on the families’ assessed needs
- Providing educational advocacy alongside families
Who Qualifies for Free Case Management Services?
Eligible Family Types:
Our short-term case management program serves Wisconsin families who are:
- Post-adoptive families with children who joined your family through adoption
- Post-guardianship families caring for children under legal guardianship
- Reunified families where children have returned home after out-of-home placement
Program Requirements:
- Child must be under 18 or still in high school
- Family resides in Wisconsin
- Primary concerns relate to your adopted, guardianship, or reunified youth in the home
- Both parents/guardians willing to participate for up to six months
- Commitment to ongoing meetings, with at least one in-person visit after orientation
How Our Case Management Process Works
Step 1: Initial Contact and Consultation
Contact our team to discuss your family’s needs and learn about the program. We’ll schedule a consultation to explain program eligibility and how case management may support your family.
Step 2: Application
After a family’s application is reviewed and approved as a good fit, the next step is scheduling orientation.
Step 3: Orientation and Family Assessment
Your dedicated case manager will conduct a comprehensive assessment to understand your family’s strengths, challenges, and goals.
Step 4: Personalized Support Plan
Together, we’ll create goals to address your family’s specific needs and connect you to appropriate resources.
Step 5: Ongoing Support and Coordination
Your case manager provides regular support, assists in identifying services, and helps you navigate challenges as they arise.
